How to Make a Gift

Outright Gifts

This is the most preferred gift form because of the immediacy of its usefulness in the foundation's work. Outright gifts may be made in the form of cash, securities and gifts of saleable property. The foundation office and our brokerage firm, Ascension Investment Management, can assist you. Gifts of paper stock or electronic transfer can be made.

Electronic Stock Gifts

You may make gifts by electronic transfer of stock through our account at Ascension Management Investment. Please contact the Ascension Treasury at and review contact and account information instructions on this stock form or mutual fund form. Transfer stock to DTC number 0226 and instruct to credit the St. Elizabeth Hospital Foundation. It is helpful if you can notify the foundation of your plans by contacting us at (920) 831-1475 or

Qualified Charitable Plan

Name the foundation as beneficiary of a qualified charitable plan (IRA, Tax-Deferred Annuity, 403B, etc.). A simple beneficiary form or change of beneficiary form is required. Contact the foundation office at (920) 831-1475 or with any questions.

Charitable Lead Trust

A Charitable Lead Trust (CLT) provides the opportunity to make a major annual gift from the trust to a nonprofit for a limited period of time, following which the trust will pass to family members. CLT’s can be established during your lifetime, or they can be provided for in a will, providing a payment stream to the nonprofit for a term of years and significant tax savings for your estate. Family members eventually receive all the assets in the trust.

Gifts of real estate or life insurance require information and documentation. Please gather documents related to the type of gift you are considering and contact the foundation office at (920) 831-1475 or

Bequest Provision

The foundation can provide you or your advisor with the legal name and preferred language for including the foundation as a beneficiary of your will or trust. The foundation welcomes non-cash contributions of items that directly benefit patients, as long as the department accepting the items determines the contribution is appropriate. Please contact the foundation to ask about making gifts in-kind. When accepting an in-kind contribution, the foundation will complete its section of IRS Form 8283, which is used by those who itemize on their income tax returns. This form allows the donor to claim tax deductions for non-cash charitable gifts. It is the donor’s responsibility to indicate the value of the gift and to fill out various sections of the form, depending on that value.
