Meeting our Mission


Our mission is to live out the healing ministry of Christ with a special emphasis on those in need, by generating, stewarding and distributing funds to enhance the quality of Affinity Health System's services and programs and other community health care initiatives.

St. Elizabeth Hospital Foundation serves as the philanthropic arm of Affinity Medical Group clinics, Calumet Medical Center and St. Elizabeth Hospital, with a primary purpose of raising and distributing funds for equipment, projects and programs in Affinity's northern service region, with an emphasis on serving the poor.

Each year thousands from our community turn to Affinity Medical Group Clinics, Calumet Medical Center and St. Elizabeth Hospital for their medical care. They are cared for by amazing teams of doctors, nurses, support staff and volunteers, who go above and beyond to ensure that individuals and families in our community get the best quality care, close to home. Your gift to the Foundation helps each and every person that enters our doors. You make a difference to health care delivery in our great community. Thank you!


It's Personal Capital Campaign

The campaign to support the renovation of St. Elizabeth Hospital continues!  As of April 20th, $5,304,024 has been raised from generous employees, physicians, grateful patients, businesses, and community members.  If you are interested in joining the over 1,000 individuals and businesses committed to this project, visit us today at www.itspersonalcampaign.org!

Renovation of the Women and Families Center began with the creation of our new Breast Center now located on the first floor of St. Elizabeth Hospital and will end with expanded and redesigned birthing suites. Our new BirthPlace Center, including our existing state-of-the-art neonatal intensive care unit, will create much needed space. New mothers and families will have the privacy and space needed to nurture and bond with their child. We know the importance of creating a family-centered care environment and the impact it will have on Fox Cities families.

25 for 25 Charity Challenge

We are the proud recipient of a $2,500 grant from the Community Foundation for the Fox Valley Region's 25 for 25 Charity Challenge.  Funds from the grant were directed to support St. Elizabeth Hospital’s Child and Adolescent Behavioral Health Unit with the purchase of supplies for the health and wellness programs.  These programs are an integral part of the therapeutic services included in each child’s individualized treatment plan. Thank you for your support!!


 Join us on Monday, August 6, 2012, at Butte des Morts Country Club as we raise $100,000 for a new Mother Baby and Family Room for the St. Elizabeth Hospital Birthplace Unit as part of the It's Personal, Continuing our Caring Commitment capital campaign. This is our largest target yet and we need your help!

Foundation Annual Report

In 2010, we began producing our annual report, On A Mission, online to provide immediate access to St. Elizabeth Hospital Foundation information in an ecologically and financially responsible manner. Please review our annual report to learn how your generous gifts helped Affinity Medical Group clinics, Calumet Medical Center and St. Elizabeth Hospital serve the medical needs of individuals throughout the Valley.


Grateful Patients Making a Difference

Patients like Elizabeth who are grateful for exceptional, personalized care, oftentimes show their gratitude by making a gift in honor of their caregiver.  What a wonderful way to give back and ensure others receive that same outstanding care!  If you would like to honor a special nurse, doctor or staff member for their care, you can make a gift online or call the Foundation office.  Your caregiver will receive a note that a gift was made in their honor, and any personal words of thanks you share will be included.  


Blankets Needed - You Can Help!
Kaukauna Key Club Blanket DonationSt. Elizabeth Hospital departments are in need of blankets for their patients.  Blankets of all colors, shapes and sizes, from fleece to wool, are used to keep patients warm and comfortable while receiving their care.  If you have new blankets you would like to donate, please call or email us at the number or address below.  Thank you!

Did You Know?
  • Affinity Health System, Children’s Hospital of Wisconsin and ThedaCare have created Catalpa Health to address community mental health needs, providing outpatient child and adolescent mental health services.
  • SEH's Blood Bank has been granted AABB accreditation through 2014. Accreditation follows an intensive on-site assessment by specially trained AABB assessors and establishes that the level of technical and administrative performance within the facility meets or exceeds the standards set by AABB.
  • Calumet Medical Center's Progressive Agricultural Safety Day will be held Tuesday, July 17, 2012. Register today!
